This Website Privacy Policy describes how Caventa Health LLC, d/b/a Caventa ("Caventa," "we," "us," or "our") collects, uses, and shares information through caventahealth.com and related public-facing website pages and forms (the "Site").

1. Scope

This Policy applies to information collected through the public Site, the Fillout/Zite waitlist, and comparable public non-patient informational forms. These forms are designed to collect only basic contact and eligibility information and not symptoms, diagnoses, medications, medical history, or other clinical intake. Clinical information collected after a person enters Caventa's patient enrollment or care workflow is governed by Caventa's HIPAA Notice of Privacy Practices and applicable law.

2. Information you provide

Caventa may collect public-site/waitlist information such as name, email address, telephone number, state, age/eligibility information, and basic scheduling/contact information. Do not submit symptoms, diagnoses, medications, medical history, or other clinical information through public waitlist or meet-and-greet forms.

3. Information collected automatically

Depending on Site configuration, Caventa and service providers may collect limited technical information such as IP address, browser/device information, pages viewed, referring page, approximate location derived from IP address, and security/diagnostic logs. Cloudflare is used for public-site security/analytics. Google Ads/conversion measurement is limited to the public caventahealth.com homepage and measurement of clicks on the Join Now, Schedule Free 15 Min Call, and Join Waitlist buttons. Advertising tracking is not intended to continue onto the linked waitlist, scheduling, enrollment, portal, or other patient-facing pages.

4. How we use information

Caventa may use public-site information to operate, secure, troubleshoot, and improve the Site; respond to inquiries; manage the waitlist and enrollment communications; determine whether Caventa is accepting patients in a relevant state; measure public-site advertising performance; comply with law; and protect Caventa, patients, users, and the Site from misuse or security threats. Caventa does not sell waitlist information and does not use patient/clinical information for third-party targeted advertising.

5. Service providers

Caventa may share Site information with service providers that support hosting, security, analytics, forms, communications, and related functions, subject to appropriate privacy/security controls. PHI is handled through approved clinical workflows and vendors under Caventa's HIPAA Notice of Privacy Practices and applicable business-associate arrangements.

6. Cookies and tracking

Caventa may use cookies or similar technologies for site security, functionality, Cloudflare analytics, and the limited public-homepage advertising measurement described above. Caventa does not intend to deploy advertising pixels, remarketing, enhanced conversions, audience-building, or similar behavioral tracking on linked waitlist, scheduling, enrollment, portal, authenticated, or clinical pages. A material change to that configuration requires privacy review and an update to this Policy when appropriate.

7. Data retention

Caventa retains waitlist/contact information only as long as reasonably needed for the disclosed purpose, legal/compliance needs, dispute resolution, or security. Retention by service providers may differ under their terms. Clinical records follow Caventa's medical-record and HIPAA retention policies.

8. Security

Caventa uses reasonable administrative, technical, and physical safeguards appropriate to the nature of the information and systems used. No method of electronic transmission or storage is completely secure.

9. Your choices and applicable rights

You may ask Caventa to correct or delete public waitlist/contact information that is not required to be retained, subject to applicable law and legitimate business needs. Additional rights may apply under state or federal law. For rights concerning medical records or PHI, use the patient portal or contact Caventa's Privacy Officer at the information below.

10. Texas health-record requests and complaints

For Texas consumers/patients, Caventa provides the following instructions: (1) to request your Caventa health records, submit a written request through the Elation patient portal or email info@caventahealth.com and include enough information for Caventa to verify your identity and locate the requested records; (2) to contact Caventa's licensing authority, visit the Texas Medical Board website at tmb.texas.gov or call the Board's complaint hotline at 800-201-9353; and (3) to submit a Texas medical-privacy complaint, you may contact the Texas Attorney General's Consumer Protection / Patient Privacy resources. Caventa will process record requests under applicable HIPAA and Texas requirements.

11. Children

The Site and Caventa's current practice are intended for adults age 18 and older. Caventa does not knowingly use the public Site to enroll minors as patients.

12. Changes

Caventa may update this Policy as practices, vendors, or law change. The current version and effective date will be posted on the Site.
